Registration

You can either fill out your registration information electronically, or press the links at the bottom of this page and print the forms, then send them via U.S. Mail.


Early Registration date is August 9th, 5-7pm. Please see this for more info.

Then, once back to hockey night occurs, you can pay your registration fees via check or cash.



Or you can print the forms below and send via snail mail. There are (5) forms in all that need to be submitted to Shawano Hockey.

1. SHL Registration Form-1 form per player.

2. SHL Work or Pay Contract-1 form per family.

3. US Hockey Consent to Treat-1 form per player.

4. US Hockey Registration Certification-1 certificate per player.

4. Concussion Awareness Acknowledgement-1 form per player.

Twelve Days of Christmas:

Please note that these raffle tickets must be paid for with registration. Mite Parents pay $75 for 15 books of tickets and they are sold for $5 a book. Squirts, PeeWees and Bantam Parents pay $125 for 25 books of tickets and they are sold for $5 a book. You keep the money that you raise by selling the tickets.

Tickets can be picked up at Back To Hockey Night. Please check back for a date and place.
